We add the charge lines that can be read and compare them with the total printed on the same page. We do not invent a contract rate.
The review reads the file you upload. A hospital bill is marked valid only when the text shows a provider, medical-bill language, and amounts. The charges are the lines that can be read. The total is the total printed on that file.
Two findings can create a dispute amount: the same line printed more than once, and a printed total that does not match the sum of the charge lines. A reunion flyer, a notes document, or a photo we cannot read does not get invented charges.
We do not reprice a bill to Medicare, to a PPO contract, or to a reference-based schedule. Those rates are not on the page unless you upload a document that states them. Contract-style repricing for a health plan is a different service, and it is not what this review does.
